CASE MANAGER SPECIALIST
Working Unit: Adult MH Case Management
Hours: 8:00 a.m. – 5:00 p.m, Full time; Flexible

General Description
Under the general supervision of the program manager with some latitude for independent judgment, the case manager will assess the needs, obtain and coordinate services for individuals in the Adult Mental Health program. Primary responsibilities include, but are not limited to, case management, skills training, screening, crisis intervention, developing comprehensive service plans, implementing and monitoring of services. Completes necessary paperwork and documentation for above responsibilities.
Qualifications:
A bachelor or master’s degree from an accredited college or university with a major in psychology, social services or related field required and be credentialed as a Qualified Mental Health Professional (QMHP). A minimum of 1 to 2 years experience in mental health and mental retardation field with crisis experience strongly desired. Knowledge of mental illness, mental retardation, medications, resources and commitment procedures essential in performing a wide range of crisis duties. Professionalism in dealing with community officials and resources a must with good oral and written communication skills. Computer skills essential. Must be able to work in stressful situations. Position requires travel within a seven county area while on call. A valid Texas Driver’s License with driving record to comply with CFLR is essential. If selected for this position, employee must give CFLR authorization to request copy of driving record as well as all required prescreens and background checks. Must be able to satisfactorily complete required staff development training.

General Description
Position is under the supervision of the Custodial Manager and with some latitude for independent judgment. The Custodian’s work involves performing moderately complex janitorial work and instructional work in cleaning, sanitation, as well as, coordinating the cleaning and sanitation of contract buildings and facilities. Must conduct inspections of buildings; instruct janitorial crew members and inventory and work activities. Will perform related work as assigned.
Qualifications:
Graduation from an accredited high school or GED is required. Knowledge of cleaning techniques and procedures are preferred. Also, have skill in the use of custodial materials and chemicals and in the operation of custodial equipment.

General Description
Position is under the supervision of the Chief Financial Officer and with some latitude for independent judgment. The Custodian’s work involves cleaning and caring for the buildings and maintain custodial equipment. Such functions will include but are not limited to: sweeping, mopping, dusting and cleaning windows, light fixtures, or carpet. Performs rest room maintenance, replenishing supplies and applying cleaning chemicals, as appropriate, as well as cleaning and disinfecting areas such as rest rooms, offices and other facilities. Will also clean office furniture, walls, windows, and floors; and empties trash. May perform minor maintenance and repair work on custodial equipment. Will perform related work as assigned.
Qualifications:

General Description
Under the supervision of the Respite Program Supervisor, position provides responsible guidance and care to individuals in need of respite on a short-term basis. Work involves record keeping of consumer behavior and progress. Employee will coordinate services and progress reports with Service Coordinators. Position requires providing training in daily living skills, or other areas, according to Individual Care Plans. Position requires close contact with consumers. Applicant must be able to lift in excess of 45 pounds and perform other physical and mental activities normally associated with care giving.
Qualifications:
Graduation from a standard high school or GED equivalent, with a minimum of six months working with persons with disabilities preferred. Good communication skills required, both verbal and written. Fundamental computer skills are essential to enter and retrieve data from the system. Must have a valid Texas driver’s license and driving record in compliance with CFLR policies. Under the supervision of the Children’s Services Director and with some latitude for independent judgment, the Family Partner Worker assesses the need and provides support to help the guardians of children and youth. Will provide peer mentoring, education and support to the caregivers of a child who has a serious emotional disturbance and is receiving services from the Center. Good interpersonal skills and ability to communicate effectively both verbally and in writing are essential. Will also assist case managers and supervisors with current client issues.

General Description
Performs tasks involved in planning and providing activities, therapies and training programs for individuals with intellectual or developmental disabilities. Involves accurately recording and communicating consumer progress and behavior. Assists the consumer with daily maintenance of home-like environment. Must possess basic knowledge of nutrition, housekeeping, meal preparation, personal hygiene and basic first aid. Position requires flexibility of schedule, and the capability to learn and adhere to ICF-MR guidelines. Position requires operating a Center vehicle to transport consumers. Position is under the supervision of the QMRP or Assistant QMRP, with some latitude for independent judgment. Working closely with consumers, position requires lifting in excess of 45 pounds, sitting, walking, bending and full range of physical and mental activities normally associated with care giving.

General Description
Position is under the supervision of the MR Program Supervisor with considerable latitude for independent judgment. Provides case management services to individuals receiving services through IDD Service Coordination including developing and monitoring the implementation of the individual’s PDP/ISP; coordinating and monitoring the delivery of services and services from other sources; integrating various aspects of services. Work requires aggressive intervention and frequent in-person, sometimes long-term contact with individuals, families and service providers. Position requires use of a Center vehicle.
Qualifications:
Bachelor’s degree from an accredited college or university with a major in social, behavioral, or human service field is preferred. Prefer experience working directly with individuals with disabilities and their families. Good oral and written communication skills with ability to prepare clinical reports which meet the standards required. Bilingual a plus. Good time management and excellent organizational skills with the ability to manage large amounts of paperwork, to review documentation of work performed by direct care personnel and documentation of own work performed. Computer skills in the areas of Microsoft Word are essential. Must have a valid Texas driver’s license and a driving record to comply with CFLR policy.
