General Description

Performs tasks involved in planning and providing activities, therapies and training programs for individuals with intellectual or developmental disabilities. Involves accurately recording and communicating consumer progress and behavior. Assists the consumer with daily maintenance of home-like environment. Must possess basic knowledge of nutrition, housekeeping, meal preparation, personal hygiene and basic first aid. Position requires flexibility of schedule, and the capability to learn and adhere to ICF-MR guidelines. Position requires operating a Center vehicle to transport consumers. Position is under the supervision of the QMRP or Assistant QMRP, with some latitude for independent judgment. Working closely with consumers, position requires lifting in excess of 45 pounds, sitting, walking, bending and full range of physical and mental activities normally associated with care giving.

Qualifications

Graduation from a standard high school or GED equivalent is required. Experience working with persons with intellectual or developmental disabilities is preferred. Good communication skills, both verbal and written, are required. A valid Texas driver’s license and driving record in compliance with CFLR policies is essential. General Description

Position is under the general supervision of the HCS/TxHmL Program Manager with latitude for independent judgment. Provides nursing coverage for the HC/TxHmL program. Responsibilities include administering and monitoring the use of medications, monitoring health data and information, assisting individuals to secure emergency medical services. Incumbent must also maintain records, arrange transportation. Performs health care procedures prescribed by a physician/medical practitioner and monitors tasks delegated to other service providers. Work requires aggressive intervention and close contact with individuals, families, and service providers. Work requires use of a Center vehicle and a flexible work schedule. Qualifications

This position requires frequent, long-term contact with individuals, families and service providers. Flexible work schedule will be required as needed. Qualifications

Bachelor’s degree from an accredited college or university with a major in social, behavioral, or human service field. Prefer experience working directly with individuals with disabilities and their families. Good oral and written communication skills with ability to prepare clinical reports which meet the standards required. Bilingual a plus. Good time management and excellent organizational skills with the ability to manage large amounts of paperwork, to review documentation of work performed by direct care personnel and documentation of own work performed. General Description

This position will serve under the general supervision of the Director of Adult Mental Health Services or other designee as assigned by the Chief Executive Officer. The Tele-presenter will provide assistance to providers and consumers by scheduling appointments, explaining the tele-health process, and providing face to face services as needed to those served under the 1115 waiver and related projects. It is important that this position work toward the development of a lasting and meaningful program that will enhance the frequency and quality of services that we provide to those individuals in our three waiver regions (RHP 8, RHP 11, and RHP 13). This position will be expected to act professionally following CFLR guidelines for professional conduct. This position is limited to 1115 waiver funding.

Under the general supervision of the program manager with some latitude for independent judgment, the case manager will assess the needs, obtain and coordinate services for individuals in the Adult Mental Health program. Primary responsibilities include, but are not limited to, case management, skills training, screening, crisis intervention, developing comprehensive service plans, implementing and monitoring of services. Completes necessary paperwork and documentation for above responsibilities.

Qualifications

A bachelor or master’s degree from an accredited college or university with a major in psychology, social services or related field required and be credentialed as a Qualified Mental Health Professional (QMHP). A minimum of 1 to 2 years experience in mental health and mental retardation field with crisis experience strongly desired. Knowledge of mental illness, mental retardation, medications, resources and commitment procedures essential in performing a wide range of crisis duties. Professionalism in dealing with community officials and resources a must with good oral and written communication skills. Computer skills essential. Must be able to work in stressful situations. General Description

Under the general supervision of the crisis supervisor or appropriated staff, and with some latitude for independent judgment, the Transportation Officer serves to provide transportation of individuals in crisis situations who have been determined as needing psychiatric evaluation. Requires close contact with consumers. Assists in the safe loading and unloading of passengers. Occasionally Transport Officer may be called up on to provide one-on-one monitoring of a consumer for an undefined amount of time. Primary responsibilities include assisting crisis workers or appropriate staff as indicated and being responsible staff for assigned transportations.

Work requires aggressive intervention and frequent in-person contact with individuals, families, and service providers. Incumbent will be working closely with clients and must be able to lift in excess of 45 pounds. The Respite facility provides a safe, home-like alternative to psychiatric hospitalization for non-aggressive adults. Primary responsibilities include screening, performing developmental assessments, occupational therapy and assistive technology evaluations participating in Individual Family Serviced Plan Meetings, linking/monitoring services and advocating for the individual. Bachelor’s or Master’s degree from an accredited Occupational Therapy Program is required. Must possess a current Occupational Therapy License from the State of Texas.
