Case Manager III – Crisis Interventionist

General Description

Under the general supervision of the crisis supervisor and with some latitude for independent judgment, the Crisis and Intake Specialist assesses crisis situations, obtains and coordinates services for individuals in crisis to ensure the individual’s needs are met. Primary responsibilities include screening, crisis intervention, performing comprehensive onsite and mobile crisis outreach assessments, developing a crisis resolution with client, family members, or other interested parties. Making decisions regarding disposition of crisis re: return home, respite, hospitalization, intake and continuity of care.

Work requires aggressive intervention and frequent in-person contact with individuals, families, and service providers. Incumbent will be working closely with clients and must be able to lift in excess of 45 pounds. The work environment is such as to require sitting, walking, lifting, bending, and the full range of physical and mental activities normally associated with care giving. Prefer experience working with individuals with mental disabilities. Work schedule will require flexible hours. This position requires the use of a Center vehicle.

Qualifications

A bachelor or master’s degree from an accredited college or university with a major in psychology, social services or related field required and be credentialed as a Qualified Mental Health Professional (QMHP). A minimum of 6 months experience in mental health and mental retardation field with crisis experience strongly desired. Knowledge of mental illness, mental retardation, medications, resources and commitment procedures is essential in performing a wide range of intake and continuity duties. Professionalism in dealing with community officials and resources is a must, displaying good oral and written communication skills. Qualifications:

Graduation from a standard senior high school or equivalent is required plus experience in performing accounting functions. Rehabilitation Therapist Tech

General Description

Under the supervision of the Respite Program Supervisor, position provides responsible guidance and care to individuals in need of respite on a short-term basis. The Respite facility provides a safe, home-like alternative to psychiatric hospitalization for non-aggressive adults. Work involves monitoring individuals, recording progress, and maintaining the Respite environment. Employee will coordinate services and progress reports with Service Coordinators. Position requires close contact with individuals in mental health crisis. Applicant must be able to lift in excess of 45 pounds and perform other physical and mental activities normally associated with caregiving and housekeeping.

Qualifications:

Graduation from a standard high school or GED equivalent, with a minimum of six months working with persons with disabilities preferred. Good communication skills required, both verbal and written. Must have a valid Texas driver’s license and driving record in compliance with CFLR policies. If selected for position, must give Center for Life Resources (CFLR) authorization to send for copy of DPS driving record as well as all required prescreens and background checks. General Description

Under the general supervision of the Early Childhood Coordinator, and with considerable latitude for independent judgment, the Therapist provides occupational therapy and special instruction to children birth through 3 with developmental delays and/or medical conditions that might result in developmental delays. Primary responsibilities include screening, performing developmental assessments, occupational therapy and assistive technology evaluations participating in Individual Family Serviced Plan Meetings, linking/monitoring services and advocating for the individual. Works very closely with children and their families. Work schedule may require some flexibility. Requires use of Center vehicle and travel to home visits in the seven county catchment area.

Qualifications:

Bachelor’s or Master’s degree from an accredited Occupational Therapy Program is required. Must possess a current Occupational Therapy License from the State of Texas. Experience working with children ages 0-3. Knowledge of child development, assistive technology, developmental delay, medical conditions, and assessment preferred.
